Success Stories

Success story 1:
  • Six year old male child admitted during the covid pandemic with new onset seizures following two days of fever, altered behavior, insomnia, involuntary movements involving limbs,
  • perioral region – with normal MRI brain, normal CSF analysis, diffuse slowing in EEG, - diagnosed as Autoimmune encephalitis, diagnosis of NMDAR encephalitis confirmed by CSF autoimmune panel, successfully managed with immunotherapy. Child recovered without any sequelae.
Success story 2:
  • 10 year old presented with acute onset of quadriparesis with bilateral papillodema – diagnosed as idiopathic intracranial hypertension. Managed successfully with lumbar puncture and antiodema measures. Child started walking independently two months later .
Success story 3:
  • Successful management of acute necrotising encephalopathy of childhood (ANEC) with immunotherapy in a child with an acute onset of fever, seizures and altered sensorium. Child recovered without any focal deficit.
Success story 4:
  • Management of acute flaccid quadriplegia with hyperammonemia, correction given, quadriplegia improved. Evaluation revealed the child to be having urea cycle disorder.
Success story 5:
  • 4 years old presented with acute flaccid weakness of all 4 limbs. Initially worked up as a case of GBS. Serial serum potassium values were found to be low persistently despite potassium correction. Child also had hypertension. Endocrine opinion showed normal cortisol, low rennin and high aldosterone.
  • Ct abdomen with contrast normal. So primate hyperaldosteronisum was diagnosed. As part of treatment protocol, adrenalectomy done. Presently on follow up with no further episodes of flaccid weakness noted.
Success story 6:
  • 2 years old girl presented with acute ataxia with areflexia. Initially diagnosed as Incomplete Miller Fisher syndrome and started on immunotherapy. USG abdomen revealed neuroblastoma. Later child developed opsoclonus. Upfront immunotherapy was started as concurrent iv immunoglobulin and methyl prednisolone.
  • Tumour was successfully removed by pediatric surgery department. N-myc amplification was positive, so tumour strafied as high grade. Received 4 cycles of cyclophosphamide and monthly doses of iv immunoglobulin for 1 year. Child recovered completely.
Success story 7:
  • 8 years old girl admitted with COVID, managed for MIS-C. Had to be ventilated twice and developed recurrent lung collapse in post covid period. RNS revealed decremental response. Serum acetylcholine receptor antibody titre was high. Diagnosed as covid trigerred autoimmnune myasthenia gravis. Managed with ach esterase inhibitors, steroids and immunosupressants and underwent thymectomy.
  • She developed multiple episodes of myasthenic crisis despite multimodal treatment. Rituximab was tried. There were no further episodes of crisis and she is currently maintained on acetylcholine esterase inhibitor and low dose immunosuprasants.
